He actually had pretty good hygiene, considering what I was lead to expect. Most of the dirt on him was actually DIRT, and not human-waste related. I've actually heard "failure to thrive" used to describe adult patients post-bariatric surgery. Thus, it was really much tougher to have to tell HIM that it had been decided that he could no longer care for himself.
